More Than $250,000 in Proposed OSHA Fines
Reduced to $8,000
A Kentucky company faced more than $250,000 in proposed OSHA fines following photographic evidence of unsafe equipment practices. The citations included OSHA 300 log issues, training violations, and operational violations, resulting in eight violations with potentially significant financial consequences.
Resqhawk Services reviewed the citations, documentation, training records, and circumstances surrounding the violations. During the review, Tim identified violations that had been improperly cited and worked through the regulatory process to provide supporting documentation and negotiate appropriate reductions.
The original eight violations were reduced to three, and proposed fines exceeding $250,000 were reduced to $8,000. The company paid the final assessment, and the matter was closed and satisfied by Kentucky OSHA.

Finding the Root Cause After Serious Arc Flash Incidents
Following serious arc flash incidents at multiple sites, organizations needed to determine exactly what happened, identify contributing factors, respond appropriately to regulatory investigations, and reduce the risk of another incident.
These investigations involved more than equipment. Employee actions, company procedures, contractor involvement, training requirements, electrical safety standards, and regulatory compliance all had to be examined.
Resqhawk Services was brought in to support root cause analysis and the investigation process. The work included reviewing standard operating procedures, employee training, safety processes, contractor involvement, and compliance with applicable safety requirements.
Tim also served as a liaison during regulatory investigations, helping organizations understand what went wrong, identify corrective actions, strengthen procedures and training, and address potential regulatory consequences.
The investigations identified the factors contributing to the incidents and gave the organizations a clear path toward corrective action. In incidents where employee failures to follow established company safety protocols and applicable electrical safety requirements were determined to be contributing factors, the findings helped distinguish individual procedural violations from failures of the company's established safety systems.
The resulting analysis also provided opportunities to strengthen SOPs, training, contractor controls, and future incident-prevention measures.
A Facility Closure Revealed a Serious Compliance Gap
While supporting the decommissioning of a company facility, Resqhawk Services discovered a significant issue: the site's environmental health and safety processes accounted for biological materials but failed to properly address hazardous chemicals, flammable materials, hazardous waste storage, and disposal.
The existing compliance program had left the company exposed to potential notices of violation, regulatory action, litigation, and possible closure-related complications.
Resqhawk identified the missing hazardous-material and hazardous-waste requirements during the decommissioning process and helped address the materials before final site closure.
This included evaluating how regulated materials had been stored and disposed of, identifying the appropriate hazardous-waste pathways, and helping ensure the materials could be properly removed from the site.
The compliance gaps were identified before local regulatory agencies discovered them during the closure process, giving the organization an opportunity to correct the issues and mitigate its regulatory exposure while the facility was being decommissioned.
A compliance program can look complete on paper and still contain dangerous gaps. Experienced, hands-on review can uncover risks that routine documentation alone may miss.
From an 8,000 Sq. Ft. Facility to 45,000 Sq. Ft. and Back in Operation in Three Months
A growing biotechnology company producing lab-grown leather from bovine cells needed to move from an approximately 8,000-square-foot office and laboratory facility, including a 2,500-square-foot lab, into a new 45,000-square-foot location.
At the same time, the organization was growing rapidly from approximately 30 employees to 120 employees.
The new location needed much more than laboratory space. Infrastructure, process systems, offices, permits, training, signage, workplace systems, and operational requirements all had to come together so research, development, and manufacturing could resume.
Resqhawk supported the transition from facility planning through operational readiness.
